    - Added #668814: ajaxify hierarchy selectors.
    - Fixed a bug that caused new root layers to be added if "Add item" was clicked for the first child layer of another layer.
    - The original and edited indexes are now available as form value elements in index_form_add(), so they can be used by modules that hook into the form.
    - The add index form no longer displays the new page path after a form update, but the existing path instead.

    Fixed bug in the index add form caused by the newly added JS that cleaned up button values. The bug prevented the form from being updated properly after a user clicked the "Add item" or "Remove" buttons.
