  Changes to faclitate adding the mlm element to arbitrary forms:
  - Ensure that the original form's #submit callback is included when you add an
    mlm element
  - Hide mlm_mail element if it's duplicated by an existing mail element in the 
    same form.
  - If a user has been created (e.g. on the user_register form), respect the 
    newly-created account as the subscribing account.
