This patch removes the unused "Title for 5:" textfield from settings. There is no #5 displayed in the rating system.

The module at HEAD now allows admins to set the max rating to 3, 4, or 5. If the admin sets it to 5, then the fifth rating title is relevant. Rating titles that are above the maximum rating are described as such.
