You don't give anyone anything much to go on.

Describe how you upgraded, for example.

I forgot set the config file to be modified.

This isn't the most helpful thread, error and solution lack any detail helpful to anyone else unfortunately.

I'm getting a similar error now upon running update.php. I have the $update_free_access = TRUE; in settings.php. I followed both the direction in UPGRADE.txt and http://drupal.org/node/550130 but I am seeing the error below.

PDOException: in system_update_files_database() (line 2294 of /var/www/drupal7.nuim.ie/modules/system/system.module).

I am upgrading from a stand alone site with everything in the default folder over to a multisite environment, this shouldn't really make a difference. I updated file paths in the files table to point to sites/domain.com/files rather than sites/default/files

I've done nothing else special and have all non-core modules turned off, so don't have any other useful info I can think of.

Before running update.php did you ensure all your contrib modules and core was updated to the latest d6 version?
