I need a "log-in" menu item (Navigation or Primary link) that opens the log-in page, and then changes to "log-out" after a successful log-in. This would keep the viewing area nice-and-tidy for sites where visitors are not able to create an account, but where members need a visual log-in/log-out menu item.

have you taken a look at the loggintoboggan module ? I believe the feature you seek is built into it or something very close that may be easy to manipulate to do exactly what you want.
Exactly what I wanted - except simpler/more consise
This will work just fine, but I wanted something even simpler and concise.
My application: member only accounts, where "Only site administrators can create new user accounts."
(1) I want a menu item (both ways), not a block/block or block/menu (the current options for "Display the 'logged in' block" selected or unselected).
I want a menu item "login" that changes to "logout" after login.
(2) I want to see "login" instead of "login/register" (I should be able to find the place where I can edit)
