Remove wrapper DIVs around Insert button when multiple styles are not available

Right now we output a DIV around the "Insert" button even when there are no other styles to insert. This is unnecessary markup and (more importantly) makes the "Insert" button show up on a different line than the "Remove" button. This patch removes this extra wrapper if the drop-down list of styles is also not shown.
