First of all thanks for this useful little module.
Something I wasn't expecting was that if you use PHP to generate dynamic markup then it will not display unless you put some static markup as well. This is not a problem - you can put something innocuous like and then the PHP markup will display p it's just a little unexpected. This may save someone a little bit of time. Maybe in a future release someone may want to consider checking for the presence of either of these text fields and display either or both as appropriate?
