PHP streaming not working with JW Player

I've spent many hours trying to get FlashVideo + PHP streaming + xmoov + any video player working.

But in the end it still required a patch to flashvideo_streaming.module for JW Player to recognize the streamer URL. I couldn't get it working with any other players.
