Issue Summary
I don't know how difficult this is but it is something that makes sense to me, as well as being a client request.
the client wants the form to have 2 fields, Email and Name.
Reasons are
1. to be able to send emails with a name
2. so client can tell who is subscribing (they meet most clients)

This feature was previously in the module, but then was removed when the CiviCRM APIs changed.
I am not actively using this module in any of my projects, but will try to look into this in the next few months.

You can do this with the webform civicrm integration module. :)

I am strongly considering setting this to "won't fix" since I only plan on having 1 more release of this module probably, and I am going to make this module the bare minimum of features so that it will only be recommended in the case where people don't need Webform integration.
