Anonymous form always subscribes to the first list when multiple lists are available. Error: "Email Address field is required"

Hi,

Im still having a similar problem as reported AND fixed here http://drupal.org/node/810616

Modules details: Im using the latest version of mailchimp 6.x-2.4, drupal 6.20. I have 2 lists in mailchimp, both of them are "Opt-out" and roles allowed are: anonymous and authenticated.

Problem: Everything works fine when i put 1 list on the page (using panel). When i put 2 lists in 1 page, subscribing to the first one is fine but subscribing to the second list gives me errors "Email Address field is required." (see screenshots). I tried to put the list blocks into separated regions but it still checked if the first list has been subscribed.

The bug has been report in the ticket above and it was fixed but somehow i still got this problem. Does anyone else have the same problem or just me? Much appreciate if anyone can help me out with this, its kinda urgent for me.
