Since filter_list_all() generates $filters[$module .'/'. $delta] with slashes, the field attributes look like this...

name="filters[adsense/0]" id="edit-filters-adsense/0"

which I'm pretty sure is invalid. Should filter_list_all() use dashes instead or should we move this to form_clean_id to replace forward slashes with dashes for just the IDs? Let me know and I'll roll something.

This should be fixed in Drupal 7, but cannot be fixed in earlier versions, as several contributed modules and themes depend on those attribute values.
