There was a serious bug in the database schema code for beta1, so if you installed it without having had Premium installed previously, the database tables would not be created correctly.
There's no easy way for us to fix it, as the error produces different results based on your database configuration. If you were affected by this, we recommend you backup all tables in your database named premium_* and uninstall the module and reinstall it afterwards, so you'll have a sane database layout.
