Missing "Add another item" on latest stable version ?

I installed following modules
cck, filefield, filefield_sources, image_resize_filter, imagecache, imagefield, insert, jquery_ui, jquery_update, views, wysiwyg (cckeditor), wysiwyg_imagefield

From the demo youtube video, there has a button "Add another item" for multiple images upload at once. But I could find this button on my installation.

Another issue is, when creating a new content, the wysiwyg_imgefield popup window's size is show correctly however after adding images to the content and open wysiwyg_imgefield again, the popup window's size height will be larger.

I found that the more images inserted in the content, the popup window's height will be more larger
