I just upgraded my yubikey module and now I get the following error whenever I try to do anything with my Yubikey:
YubiKey OTP validation failed with message: NO_VALID_ANSWER

I setup a new API key and API ID.

Any thoughts?

This modules requires php >= 5.2. I'm adding a check to the .info file. Also this can happen when the SSL Certificate is not valid. What PHP version do you have and what servers are you verifying against?

I have the same problem
I use php 5.3 and the yubico validation servers.
