Would it be possible to define three (ore more) sizef of user-pictures?
This way there would be raly small pictures for comments, bigger pictures for the buddylist, and realy big ones for the profile.

Have a look at the imagecache module. You can define three presets and then modify your templates so that they are used in the right context. I use this to display a fullsize image in the profile and small size pictures in the forum.

1. New features are only accepted in the 7.x version now.
2. ImageCache does exactly this very well, so there's really no point in having this in core.
