Questions on survey showing duplicates

- revised Admin UI - users can rate multiple choices - added automatic none choice checkbox - added css and js files
- admin can now choose how many blank rows in adding new questions/categories/choices for each content type same with fivestars option
- please reinstall database if necessary otherwise just use update.php
