Add options to disable automatic creation of address from billing/delivery (but still default on) - patch

Issue Summary

Hi,

I would rather not create addresses based on the user's billing address, as I use uc_addresses just to manage shipping addresses. There's no current way to stop uc_addresses from trying to create an address from billing address though.

Here's a patch to add two additional options to allow toggling creation from billing and shipping.
