Fix conflict with book module.

even though the outline module can be used instead of the book.module, with Drupal 5, book.module is still in core, and used by many people. Therefore, the outline.module must play nicely with it.

The patch fixes some name conflicts.

Later, an *optional* upgrade path from book.module will be offered to those who desire it.
