Probably the same as #1091932: External links opening in both windows. Google Analytics module (if you're using it) is causing some pretty annoying problems right now.

Oh, hmm. And the little extlink icons are still showing up on the page? Are you sure that the setting for "Open in new window" is still enabled at admin/settings/extlink?
