Moderation reasons work now only in forum as it checks user_access('edit any forum topic') before showing off th list of moderation reasons to choose from. The documentation is misleading and implies user_access('administer content') is needed.

Work for the most common node types with minimal permissions, e.g.:

forum => edit any forum topic
any node type => administer content
etc…
