The attached patch will add a token [primary-termpath-full], which will build the term url based on the term hierarchy. This can be used if the taxonomy url alias does not contain the term hierarchy, but the node url structure requires it.

I have re-created the patch against the 6.x-1.x branch and fixed a couple of code styling issues with the previous patch.
Tested applying to 6.x-1.2 tag and worked fine.
