Hello there,
I originally opened this issue in the og_panels issue queue, but I now realise it is happening even when I have og_panels disabled.
Every time I flush caches (using admin menu flush cache tables) panels stop showing site wide and I get a "page not found" on my front page.
To get them to re-appear I need to disable og_panels module and re-enable it OR disable mini-panels module and re-enable it. I imagine disabling panels module and re-enabling it would also do the trick.
Any suggestions as to how could I go about investigating this further would be very welcome, as I'm stumped and it's causing me a bit of a headache dev wise.

knic commentedAnybody? My next step would be to uninstall panels, OG panels and re-install them. But then I'm going to have to recreate all my panel pages, minipanels etc, which I'd rather not have to do as I have a ton of them.
Anything else I could try here?

knic commentedUpdate: It seems that saving any setting anywhere (even without making changes) brings things back to life until the cache is flushed again.
What I've tried so far:
Disabling all modules with ctools dependencies and then using the uninstall option to remove Ctools and re-install it. Had no effect.
I'm using the latest dev releases of Ctools and Panels. Could really use a hand here. I don't know what to try next, apart from serially disabling modules and testing each time. Our site is almost finished and this is a real breaker at the moment. It seems similar to http://drupal.org/node/979912 (Panels cache getting stuck, getting white screen on my homepage panel) although for many in that issue, clearing the cache a second time brings things back to life. In my case that's not so.

knic commentedWrong call, apologies. My issues are being caused by bugs in the Purl (Persistant URL) module, so I'll take them there.

AntiNSA commentedit breaks my panels to. I can not edit panels on latest dev after flushing cache in admin menu. drush cc works ok

Letharion commented#3 clearly states this is not a Panels issue. #4 Provides no additional information beyond what is already available, so I'm resetting the status.
