When a user goes to "build my page" they find the categories listed but no way to select which ones to include in order to build the page. Permissions are correct.

Take a look at the 3 "define" variables, they are right under the comments at the top of the module code (the comments help desctibe what they do).

Without seeing your site in action, my guess is you have to change "MYPAGE_ALLOWPARENTS" to "yes"

Since I haven't heard back in 2 weeks, I will assume my suggestion to change the variables in the module worked.
