I have some users in the users table where login = 0, but access != 0.
user_prune will always delete them.
I think this is not the expected behaviour.

+1. I just edited my copy of this module exactly because of this. I have some users autologged by IP and I dont want them to be pruned.

Fixed in 7.x version of the module. D6 will not be fixed, feel free to use this patch
