after adding banner title, browser was redirected to non existing page

after fresh install of drupal 7 and rotating banner with all the needed modules (media and styles) and also with needed javascripts, i was trying to test it. so in the blocks configuration, i clicked to Add a rotating banner, entered the Banner title and then i was redirected to non existing url: http://my.site/admin/structure/block/manage/rotating_banner/1/configure from this moment i am not able to visit the blocks page until i uninstall rotating banner module. i tryed the same procedure twice (with fresh install of all modules), but the result is still the same. what's wrong?
