restrict_by_ip_address textfield is not large enough

I work for a large enterprise organization with web proxies and VPN's spread across the globe. I can't fit all of our IP addresses in the restrict_by_ip_address textfield because of the 128 character limit. The attached patch doubles the textfield and backing database column to 256 chars.
