Apply for Role module code installed in the usual place: /sites/all/modules/apply_for_roles.
Enabled module per readme.

No entry found in Administer || Site configuration.

No Tab found in /user

I tried install on a different D6 installation (same server). It worked beautifully.

I duplicated the error on two other servers with clones of the D6 installation that does not work. The problem seems to be specific to this particular D6 configuration. Are there any known conflicts with Views, Context, OG Access control... (I turned off OG access control and problem persists).

You could try installing Apply for Role in clean install of Drupal and then enabling the other modules one by one until you find the conflicting module.
