My installation:
Drupal 6.20
Taxonomy menu 6.x-2.9 (Weight 0)
Taxonomy Super Select 6x-1.4 (Weight 99)

I also have hierarchical_select-6.x-3.7, taxonomy block 6.x-1.0-beta6 installed but not activated.

I follow configuration instruction which seems straightforward. I put the menu under secondary link. I tried "free-tagging" option as suggested in other issue posts, although I don't desire to use free tagging.

But, whichever configuration I tried, I can't find a way to make the checkbox/radio button show up.

Can anyone offer any suggestion of what could have been wrong?
