I've granted the following perms in a book to one role:
Edit pages/Delete pages/Add child pages/Edit outline/View this book

The primary entry is not displaying in books block for anonymous users but if they search for the title of a child page at the third level deep it will come up in results and access they can still access it.

2nd and 4th level pages are not accesible though.

6.x is now "end of life" so unless a community-produced patch can be posted here, this issue is a non-fixable one.

If this is a 7.x-related issue as well, then this thread should be updated with more details.
