troll module
i've had very good success with the troll module (http://drupal.org/project/troll). you can ban by user name, static ip address or even an ip range. works very well and lets you know when a banned user has tried to access the site.
it doesn't look like troll has been updated for drupal 5, yet. hopefully that is not a problem.
