Alert file writing functionality can't be enabled in UI

alerts_alert_save checks for the alerts_create_file variable to determine if a file should be written, but that variable is not configurable in the UI. Similarly there is no way to set the desired file extension (alerts_create_file_extension). This patch adds options for configuring both to the alerts admin interface.
