I received this error in workbenches create content, upload media tab.

This was fixed in workbench in #1167838

I am unable to replicate this required_once error when running the workbench_media 7.x-1.0 and media 7.x-1.x. Can you upgrade to the latest versions of these modules and see if the error persists?
