ecommerce 6.x-4.4

This is a bug fixing release, and some small changes to the receipting to allow the handling of the eWAY Shared payment gateway.

Primarily this release fixes issue with the address handling of countries which do not have any states.

Changes since 6.x-4.3:

• Fix up issue with checkout for anonymous users w/caching
• remove hook_exit() and use new method to disable caching
• #1125266 by wmckinley: Fixed '&' interpreted as & on token [txn:txn-items] in rules for order confirmation.
• remove schema and table for ec_tmp.
• Fix up initialised variables in expand_credit_card()
• Fix up card type defaults as they cannot be set from the default values
• #1102838 by Observer123: Fixed State field required for non-USA country.
• fix up error for customer name required txn to be past by reference when it doesn't need to be.
• Fix up uninitialised vars in the allocation.
• Remove being passed by reference to the allocation gateway.
• Make changes to the receipting implementation so the hook_receipt_goto_url() can be forced.
