Modify Login box in header

I am trying to change the layout a bit of the login box that displays in the header. I am unable to make the changes I need to with only CSS. I am trying to figure out how to change the login box from the one on the top to the one on the bottom (see this image: http://www.theidealform.com/images/login.jpg)

Any help in pointing me in the right direction would be great.
