Forms in Ver 5

I want to include a number of 'agency' details in my site - name, address, tel No, etc. and thought that a form would be the best solution for reasons of consistency.
For Drupal 5, has anyone any recomendations of a module or way of acheiving this.
I have considered AJAX Form Builder module, but looks a bit complex - other libraries needed etc.

Any suggestions please?

I you don't want to do custom coding, try the webform module

I have tried Webform, which is ideal for adding entries, but does not display them very well to visitors.
I want visitors to be able to navigate through the records to find a relevent agency, either by taxonomy, search box, index or a combination.
I am rubbish at coding, does anyone know how this can be acheived please.
