Using Quotations in Alt or Title field breaks code

If an image's Title and/or Alt Text contains quotations "" then this breaks the code, as they are not escaped. Image still displays, but html is broken.

Noticed this because the Alt Text and Title were created automatically from my node title, and it contained quotations.
