The search doesn't work on 404 pages. Try one out here: www.madisonpubliclibrary.org/asdf . Neither "Search LINKcat" nor "Search this site" works.

This is a duplicate of #947242: Action not set on form, causing problem with search from "Page not Found" page. This has a patch for it and needs review. It has worked for a couple other people. If it works for you, let me know and I can confirm it as a good patch and make it part of the module itself.
