Flexible menu selection options

CTM currently supports explicitly setting which menus to show for a specific content type. This is useful but limiting if content types or menus are added after configuring CTM.

The following patch adds the these features:

• Whether to include all menus (bypasses CTM), no menus, include only the selected menus (Current CTM default - except no menus are selected), include all but the selected menus
• Default settings across content types under settings
