Our CAPTCHA module always denies the correct answer on the first use. When you enter it correctly the second time, then it works. For example, see the comment box at the link below.

We're using the Solve Media CAPTCHA here, but it works within the Drupal CAPTCHA. If we have the Drupal CAPTCHA math phrase, for example, the same issue is happening. Thoughts on a fix?

It seems you are using mollom and CAPTCHA module together, this is not recommended

also, version 5.x-3.x is not supported anymore
