This could be due to missing theme_abuse_report() function

Thanks for letting me know about this. I had actually packaged a abuse_report template file along with it (I guess I should have put some instructions in the README on setting that up.)

I have removed the template file and created it as a theming function within the module. That should (hopefully) do the trick.
