Taxonomy Access not working

I upgraded to 5.1. Taxonomy Access is not working because I keep getting this line in the node_access table:

nid gid realm grant_view grant_update grant_delete
0 0 all 1 0 0

Apparently it's supposed to be deleted, but it is not. This might be a node.module bug. I have tried everything I can think of including uninstalling/reinstalling all modules, reinstalling all system files, reverting the database to different stages, etc.

There has got to be a setting in the database that is causing this.

Can anyone please help?
