marcoBauli commentedyou might use one of the many Taxonomy modules out there to simply output all of your terms (or all terms in a container). Tagadelic, Sitemenu, Views just to name a few are all good starting points.

John Bryan commentedI have tried with a couple od pages created in views to generate a all taxonomy terms listing but the refine blocks remain empty. Any more detailed advice or documentation on how to get an initial "un-refined" starting page?

Bèr Kessels commentedrefine_bY_taxo shows up only on taxonomy/x/ pages.
There is a feature request that tries to fix this and make refine_bt a little more flexible in that.
