og_teampage - I don't understand

I can't seem to get this module to work...when I create a teampage, I get the following screen to fill out....but there are no blanks to fill in....it's almost like the tables weren't created. There is no option to name the team, etc.
And when I go to the node as the admin, it has tabs to "admin members" which you would think would allow to add users but it has a link to a page that supposedly would allow you to but it takes you to 'sitename/og/users//add' (the double slash is not a type-o) and obviously doesn't allow you to do anything.
Am I missing something?

see that attached pic for the screen I get when adding the teampage.
oh, and I tried to make a blog entry with the [teampage:xxx] in the body...and it did nothing - it just printed "[teampage:xxx]"

Did you set 'teampage' as a group homepage node type in admin/settings/og? Without doing this, og doesn't recognise teampage as a group.
To use the filter, you have to enable it for your input formats at admin/settings/filters !
