Filename always displayed under image, imageupload does not respect configuration

Issue Summary

Filename is always used as title - displayed bellow image, no matter what, even when use filename as title is unchecked in configuration.

imho this could be configurable on a per item bases (not just general config for every image) and user should be able to add his own title in insert image form (token support).
