E-mail notifications: enabled, profile should be set initially to off

This is a great module. Very nicely written, and good instructions.

We got a big surprise when enabling the email notifications - about 30 people with node editing privileges got hit with spam notifications all afternoon. It said that it would be enabled as a setting in profile, would be better if once it is enabled if that setting was initially off until specific profiles turned it on.
