Defaults do not match documentation / reset does not work as expected

Hello,

I'm rolling this module out on one of my sites and I'm now revisiting problems with this module for the third time. It seems like the module defaults to the *largest* size. I had to readjust everything around that setting to get the right font sizes but...

The reset button doesn't work. Well, it sort-of does. It defaults to the smallest size. So users have no idea what is going on.

When I uncheck the "reset" button in settings I get a new button to replace it. The "+X" button. I have images disabled so this produces [A- +X A] as my links. The last one, A, does nothing. Wha?
