Best way for this type of Voting

I've looked at the various voting modules but not seeing what I quite need. What I was to do is have the following;

- Several categories
- Under each category there are some suggested people
- The user can also enter free form text if a provided option does not match their choice
- The user can vote on one person in the category
- Once they have voted they cannot do so again
- The number of votes for each person in the category needs to be hidden.

I can't see a module that will do it that way, is there one?
