Weird problems with this role

I have selected that users can apply for wholesale role

in configuration screen I have checked wholesale role for

Roles:

and

Approval Roles:

On registration page I get

Choose a role:
wholesale *needs administration approval
*needs administration approval

There are 2 boxes, second one does not have even a name for the role??

What is happening with this?

Why the user does get an email to login, even tough the account is blocked? THat is misleading
