I've got a location enabled node type. For that node type an action is defined that unpublishes the node afer creation. This causes the node to have two instead of one location.

I have the actions module and locations 5.x-1.x-dev installed, and I'm seeing the same symptoms. I've set a limit of 1 location per node, but when I create a new node with information for one location, it appears twice. Editing the node reveals that it actually now has two locations with the same information.
Not sure if this is due to actions module, but I do have the same setup as you described (where the node is published/unpublished after being submitted), so it may be.

Won't fix 2.x code. Reopen if it's still broken under 3.x.
