I do not see any way to remove a node reference on the node edit form. And, when removing referenced node, an entry remains in the reference field, rendering the node with the reference field unsaveable unless the reference is replaced with another.

Nope. What I was seeing was no red x to the right of node reference entries when unlimited values are allowed and the field is not required. It works for my colleague on the same version of References. Will try the same with a fresh install later today. My guess is that this is not a References issue but a side effect from another installed module.
